This will be made available to surgical patients within 30 days of hospital discharge. Patients will have an evaluation/assessment form to evaluate the level of service provided before discharge.
We would contact surgical patients for a phone interview within two business days of discharge. In collaboration with the Hospitalist team, patients will have a face to face visit within 7 to 14 days of discharge.
This collaborative process facilitates the surgeon’s efficiency in post-operative care, improves patient satisfaction and reduces the length of stay.
